What Should a School Admission Form Ask For?
A good admission form asks for only what the school needs to place the child, reach the family and keep a proper record. That means the child's identity, the parents' contact details, the previous school, and a short list of supporting documents. Everything else can wait until after admission.
This post gives you a field-by-field list, a document checklist and the rules to keep in mind. Check your own state board or management rules before you finalise the form.
The fields to put on the form
Keep the form to two pages. Group the fields so a parent can fill them without help.
Child's details
- Full name, exactly as on the birth certificate
- Date of birth (day, month, year)
- Gender
- Class applied for
- Mother tongue and medium of instruction wanted
- Address with PIN code
- Passport-size photograph
Parent or guardian details
- Names of mother, father or guardian
- Mobile number (one that has WhatsApp, if possible)
- A second contact number for emergencies
- Email address, if they have one
- Occupation (optional)
- Address for communication, if different
Previous school
- Name of the last school and board
- Last class passed
- Transfer certificate number and date, for classes where one is needed
Health and safety
- Blood group
- Known allergies or long-term conditions
- Name and number of an emergency contact other than the parents
- Whether the child will use school transport
Other useful fields
- Siblings already studying in the school
- Preferred language for messages: Hindi, English or a regional language
- Signature of the parent, with the date
Fields to leave out
Every extra field is one more thing for a parent to get wrong and one more thing for you to store safely. Leave out anything you will not use in the first year.
- Do not ask for details that have no bearing on admission, such as parents' income, unless a specific scheme needs it.
- Do not ask for the Aadhaar number just because it is convenient. Ask only when a process requires it, and keep it private.
- Do not add long questions about the child's abilities if your admission is not meant to test the child. See the next section.
What the law says about admission
The Right to Education Act is the main law for admission to elementary classes. At the time of writing, the Act prohibits screening procedures for admission, and says newly admitted children must be placed in age-appropriate classes.
The Act also requires 25% of seats in private schools to be reserved for children from disadvantaged backgrounds. If your school takes part, the form and document list for those seats may differ from the general one. Follow your state's current notice.
Rules differ by state and by class. For your own case, check with your state education department or board.
Documents to ask for
Split the list into "needed at admission" and "can follow later." This stops a family from being turned away at the counter over one missing paper.
Usually asked at admission
| Document | Why |
|---|---|
| Birth certificate or other date-of-birth proof | To confirm age and place the child in the right class |
| Passport-size photographs | For the record and the ID card |
| Parent's ID and address proof | To confirm the contact and address |
| Previous school report card | To confirm the class passed |
Can follow within a few weeks
| Document | Why |
|---|---|
| Transfer certificate from the last school | Needed for entry into higher classes; the old school may take time to issue it |
| Medical or vaccination record | For the health section of the file |
| Caste or category certificate | Only where a scheme or reservation applies |
Do not refuse a child only because a paper is late. The Act's discussion of documentation notes that some children cannot produce such documents even when they are eligible. Record what is pending and give a date.
A note on APAAR ID
India has an academic ID called APAAR for students. According to the APAAR portal, parents give voluntary written consent through a form submitted to the school, and the school explains the programme first. It uses details such as name, date of birth, gender, parent names and a mobile number.
So a clean admission record helps later. If the name and date of birth on your form match the child's other records, there is less to correct. Ask the parent for consent separately. Do not make it part of the admission form's mandatory fields.
Make the form easy to run
- Number every form so none gets lost.
- Give parents a checklist slip that shows which documents they have submitted and which are pending.
- Write the name and date of birth once, then copy them into every record. Do not retype them in several registers.
- Keep photocopies or scans of documents with the student's file, not in a loose drawer.
- If you issue a bonafide letter later, use a fixed format. A free bonafide certificate format is a good starting point, and a student ID card format helps once the photo is in.

Where the paper form starts to hurt
A paper form works for 50 admissions. At 500, the problems begin. Enquiries sit in a diary, one family fills in the same details three times, and nobody knows which child is still waiting on a transfer certificate.
8plabs School's admissions and student records pages show enquiries from the website form, the front desk and logged calls in one list. The admission form becomes the student's permanent record, with supporting documents such as birth records, previous school papers and photographs stored with it. It also prints transfer certificates, bonafide letters and character certificates in school formats.
If you want to see the whole school package, the school software overview shows how admissions connect with fees, attendance and results.
Next step
Print the field list above and compare it with your current form. Cut what you never use and add the checklist slip.
